Specific Injuries Experience
Victoria has specific experience in working in community services with clients who have long-term community care needs. She regularly works with those who have suffered trauma leading to orthopaedic injuries such as limb and spinal fractures, joint replacements and upper and lower limb amputations. Many of her clients experience chronic pain and/or fatigue. She also works with clients who have mild brain injury and Cerebral Palsy.
Victoria has worked with a number of Brachial Plexus nerve injury and hand injury cases in her community and inpatient roles. She also has specific experience working with terminal illness, learning disability and mental health including PTSD.
Victoria is competent in the planning of care, and recommending equipment and housing needs for these client groups.
Employment Profile
Victoria’s role within Adult Community Health and Social Care requires her to provide timely and accurate assessments and reports. As part of a multi-disciplinary team, she works with adults who have long term substantial physical, neurological or cognitive difficulties.
Victoria completes full functional assessments of individuals within their home and local community. She assesses for and provides standard and specialist equipment to maintain and/or compensate for loss of independence/safety. She will consider provision of assistive technology to monitor environments for clients with vulnerabilities such as cognitive impairment, dementia and visual injuries.
Victoria will also consider provision of moving and handling equipment and training for agency staff and informal carers. She will assess the suitability of a client’s property and where this is not considered to be appropriate, she will complete a Housing Needs Report and make recommendations for a new property or minor or major adaptations to the existing property.
She is also adept at completing Care Act assessment paperwork, support plans, obtaining agreement for funding, liaising with the supporting team over the type of care package required, sourcing respite placements and long-term nursing and residential placements, exploring day care options and transport options.
Victoria also reviews the appropriateness of care packages for clients needing continuing care at home. This involves care reviews post discharge from hospital, setting up new simple and complex high-level care packages.
