Specific Injuries Experience
As a Senior Community Occupational Therapist, Simone is responsible for conducting home and hospital-based assessments to ascertain the ongoing and long-term care, therapy and equipment needs of adults with upper and lower limb amputations, orthopaedic injuries including joint replacements, spinal injuries (including spinal fractures), chronic pain conditions, and neurological conditions (such as stroke, MS, MND, Parkinson’s and Dementia) and terminal and degenerative illnesses. Simone is also responsible for reviewing the discharge cases in her area, to provide support and guidance to carers to shape the approach to an individual’s specific care needs. Simone devises long-term intervention plans that increase an injured person’s ability to carry out activities independently within their homes as well as in the community. She is responsible for managing their ongoing needs and will work with adults across a wide age range.
Simone also has experience in supporting individuals with mild to moderate brain injury and mental health conditions in conjunction with their primary injury/condition.
Employment Profile
Simone is a Band 7 Senior Community Occupational Therapist for an integrated health and social care rehabilitation service. Simone evaluates & implements highly specialised packages of care for adults with complex needs which aim to maximise rehabilitation potential. As an occupational therapist, Simone can also make recommendations for disability led adaptations for the council. This involves a high level of joint working with locality teams to facilitate the required needs. In this role she carries out assessments for those with complex long-term needs within private homes and rehabilitation facilities. These assessments cover the individual’s care needs, as well as associated equipment and home adaptation provision. As part of a multi-disciplinary team, Simone works with adults who have long-term substantial physical, neurological or cognitive injuries. Her role requires Simone to create care provision reports and to liaise with other AHPs to ensure clients have a care package that fully supports their needs.
Simone’s hands-on responsibilities also require her to carry out environmental assessments to establish long-term housing needs, including any associated moving and handling requirements and the provisioning of appropriate adaptations and equipment.
