Rachel is a highly professional and experienced occupational therapist with extensive specialised skills in the areas of long-term condition management, palliative care and neurological rehabilitation. She is an experienced case manager and has worked with clients with a range of injuries, including brain and spinal injury, amputations and complex orthopaedic injuries. In both of these roles, Rachel has experience in managing clients from the acute stage to discharge into the community and beyond.
Rachel currently works as a case manager for adults with acquired brain or spinal injury, who have complex rehabilitation and care needs. She is experienced in directly recruiting support workers and managing agency care staff. Rachel facilitates and co-ordinates support packages that enable an individual to have a safe, sociable, purposeful and active lifestyle in which they remain as independent as possible. This role involves assessing needs for case management, care and rehabilitation as well as risk assessment and development of risk management strategies. Rachel also designs, develops and monitors multi-disciplinary rehabilitation packages.
In her previous roles Rachel has managed large caseloads of clients working as an autonomous practitioner to ensure a high standard of quality care. Rachel has assessed and treated individuals with a wide range of complex and multiple needs, such as COPD, neurological conditions, orthopaedic injuries and palliative care. She carries out comprehensive assessments, using investigative and analytical skills to formulate management plans, mutually agreed goals and treatment plans.
Rachel has experience in making recommendations pertaining to packages of care within the community and home environment, including the provision of equipment and adaptations to maximise occupational performance. Whilst working in acute medicine she worked on complex discharge planning within multi-disciplinary teams which included wheelchair prescription, including mobility and transfer training for complex multi-trauma and non-weight bearing orthopaedic clients.
Rachel is an organised, reliable and hardworking therapist who works well under pressure. She is a highly experienced chair person, frequently facilitating team meetings and case conferences. Her written documentation is completed to a high standard.
